Diabetes Education Program

Empowering individuals and families to take control of their health through education, support, and traditional knowledge.

Supporting your diabetes journey with personalized care, culturally based guidance, and community-based programs that promote long-term health and wellness.

Misiway’s Diabetes Education Program is designed to help individuals better understand and manage diabetes through education, lifestyle support, and culturally relevant care. Led by a dedicated team consisting of a Pharmacist, Registered Dietitian and a Diabetes Nurse Educator, the program offers both individual and group sessions at our Community Health Centre and through outreach clinics within our catchment area.

Clients can be referred by a health care provider or self-refer, making access simple and convenient. The program provides education and care in a variety of ways to support a wholistic and culturally safe approach to wellness.

Services Include

  • One-on-one and group diabetes education and support
  • Glucose monitoring education
  • Nutritional counselling and weight loss support
  • Individualized diabetes care plans
